Is lung-sparing surgery better than extrapleural pneumonectomy?
Specialized centers have shifted decisively toward lung-sparing surgery. Propensity-matched data show 0% perioperative mortality for extended pleurectomy/decortication versus 11% for extrapleural pneumonectomy, with comparable long-term survival between the two procedures after adjusting for that perioperative difference.[2] P/D also preserves respiratory function well enough that patients can complete systemic therapy courses.
