What Are the Three Mesothelioma Cell Types?
Mesothelioma is classified into three histological subtypes based on the appearance of the tumor cells under a microscope. This classification is not a technicality — it is one of the two most powerful predictors of how the disease will behave and how it should be treated.[1]
Epithelioid mesothelioma is defined by uniform, cuboidal cells that resemble the normal cells lining body cavities. They grow in more organized patterns and behave less aggressively than the other subtypes. Sarcomatoid mesothelioma is defined by elongated, spindle-shaped cells that grow in a disorganized fashion and behave aggressively. Biphasic mesothelioma contains both cell types in the same tumor.[8]
The subtypes differ sharply in frequency. How Does Cell Type Affect Survival?
The survival difference between subtypes is one of the largest in mesothelioma. Epithelioid patients live the longest: a 776-patient multi-institutional study found a median overall survival of about 16 months, ranging from 29 months for low-grade tumors without necrosis to 8 months for the highest-grade tumors.[3] Sarcomatoid patients face the shortest survival and the poorest treatment response of the three.[4]
Biphasic mesothelioma sits between the two, and its prognosis is driven by composition: the larger the sarcomatoid component, the worse the outlook.[1] This is why a pathology report often specifies the proportion of each cell type in a biphasic tumor.

How Is Cell Type Determined?
Cell type can only be established by examining tumor tissue, which is why a biopsy is central to diagnosis. Under the microscope, the three subtypes have distinct appearances, but mesothelioma can mimic other cancers — particularly lung and, in the abdomen, ovarian or peritoneal cancers.[9]
To confirm both that a tumor is mesothelioma and which subtype it is, pathologists use immunohistochemistry, staining the tissue for proteins characteristic of mesothelial cells. Mesothelioma is typically positive for markers such as calretinin, WT1, and D2-40, which distinguishes it from the cancers it resembles.[9] The 2021 WHO Classification of Thoracic Tumours also recommends nuclear grading for epithelioid tumors, adding further prognostic detail.[3]
Because mesothelioma is rare and subtype assignment changes treatment, specialist pathology review at a high-volume center is strongly recommended. A second opinion can change the subtype — and therefore the treatment plan.[6]
How Does Cell Type Affect Treatment?
Cell type is a gatekeeper for treatment. Epithelioid histology predicts a better response to surgery, chemotherapy, and immunotherapy, which makes epithelioid patients stronger candidates for aggressive, potentially life-extending treatment.[10] For peritoneal mesothelioma, epithelioid histology is an independent predictor of improved survival after cytoreductive surgery with heated intraperitoneal chemotherapy (CRS/HIPEC).[10]
Sarcomatoid and heavily sarcomatoid biphasic tumors respond less well to surgery and chemotherapy, so treatment often emphasizes systemic therapy and symptom control.[12] Here, immunotherapy has been especially important: the combination of nivolumab and ipilimumab, FDA-approved first-line for unresectable pleural mesothelioma, provided meaningful benefit across subtypes and is a key option for non-epithelioid patients.[11]
